Memberships can be cancelled at any time with written or verbal notice. Simply let your provider or our staff know and we will assist you. We understand that patients come for many reasons, short-term, long-term, and transitional. We will always support you and any reasons to terminate treatment. Patients who leave in good standing are welcome to rejoin in the future if they need.
